Biography
Michael Mithoefer is a South Carolina psychiatrist who became the central clinical investigator of the MAPS MDMA-assisted therapy program for PTSD — the most extensive psychedelic clinical trial program in history. Working alongside his wife Annie Mithoefer (a psychiatric nurse and co-therapist), he conducted the first Phase 2 trials of MDMA for PTSD beginning in the early 2000s, when no researcher had studied MDMA in humans under regulated research conditions for over a decade.
The Mithoefers' Phase 2 results were striking: participants with severe, chronic PTSD who had not responded to years of conventional treatment showed dramatic symptom reduction, with many no longer meeting diagnostic criteria for PTSD at follow-up. These results — including in populations of sexual assault survivors and veterans — provided the clinical foundation for MAPS's Breakthrough Therapy designation from the FDA and the subsequent Phase 3 trial program.
Mithoefer's clinical approach draws on his training in Internal Family Systems (IFS) therapy and a therapeutic philosophy that trusts the healing process that emerges during MDMA sessions rather than directing it toward specific outcomes. This non-directive stance, combined with careful preparation and integration support, is a defining feature of the MAPS protocol that other MDMA research groups have largely adopted.
He has served as a senior medical director at MAPS and has been instrumental in developing the therapist training program that certifies MDMA-assisted therapists — working closely with Janis Phelps at CIIS and other training institutions. Mithoefer also worked as an investigator on early psilocybin research, connecting his MDMA expertise to the broader psychedelic clinical trial world.
