I have OCD — diagnosed, in treatment for 12 years, tried five SSRIs and done extensive ERP. ERP helps but I'm not in remission and my OCD has narrowed my life significantly. I'm curious about psilocybin specifically for OCD — I've read the Moreno 2006 pilot study and the Yale trial is ongoing. But I want to hear from people who have OCD (not just anxiety generally) and actually tried it. What happened?

I have contamination OCD. Three years ago I did a guided session at 3.5g. The OCD-specific thing that happened: during peak I encountered my contamination fear directly but from a perspective that felt completely outside of it — like I was watching the fear from a distance that made it small. For about 6 weeks after that session my rituals dropped significantly. I used that window intensively in ERP with my therapist. The rituals came back but to a baseline that's lower than before.

The SSRI interaction is a real obstacle for people with OCD. Most of us are on high-dose SSRIs because OCD requires higher doses than depression. Coming off SSRIs to try psilocybin means risking OCD rebound. My prescriber and I tried a partial taper and even reduced dose produced noticeable effects. The clinical trials are working out this dosing problem — I'd wait for the Yale results before making a decision about tapering.

Something nobody warns you about: OCD can incorporate the psilocybin experience itself as intrusive content. I had session content that my OCD latched onto as new material for obsessions. I wasn't prepared for that. Make sure your therapist has OCD-specific training and explicitly plans for this possibility in integration.
