I have moderate chronic tinnitus — constant ringing in both ears for 7 years. I've tried everything offered through conventional medicine (masking, CBT for tinnitus, no effective pharmacological treatments exist). I tried psilocybin after reading case reports. Results: the tinnitus was present during the session, then absent for about 3 days afterward. Then it returned. I'm looking for others' experiences.

The tinnitus-psilocybin connection is documented anecdotally in a small but consistent pattern in patient communities. The proposed mechanism: tinnitus involves both peripheral auditory damage and central auditory cortex changes (the brain's response to the peripheral damage). Psilocybin's DMN disruption and central nervous system effects may temporarily reduce the central sensitization component — similar to the proposed mechanism for chronic pain.

Duration of benefit in reports I've seen: typically 1-7 days. Some people report months of reduction; some report no change; a small number report temporary worsening during the session. The sample is too small and uncontrolled to draw conclusions. The Veteran Tinnitus Initiative has expressed interest in psilocybin research for this indication.

Serotonin connection: the auditory system is heavily serotonergic. 5-HT2A receptors are expressed in auditory cortex. Psilocybin's action on these receptors theoretically could affect the central amplification that maintains tinnitus perception. This is speculative — there are no published clinical studies of psilocybin for tinnitus.

Practical note: tinnitus during the psilocybin session itself can be intensified. The sound amplification aspect of psychedelics applies to tinnitus as well as external sound. This can be distressing at peak dose. Using music throughout the session reduces the attention available to notice and be bothered by the tinnitus.
