Australia became the first country to federally authorize psilocybin therapy in February 2023, effective July 2023 through the Therapeutic Goods Administration (TGA). I've been researching how it actually works in practice and there's a lot of confusion about it. It's not a free-for-all — the system is quite restrictive. Here's what I've found.

The TGA system: psilocybin is not 'legal' in Australia in the way that, say, cannabis is medical in various states. It's legal for authorized psychiatrists to prescribe psilocybin for two specific conditions: treatment-resistant depression and PTSD. The drug must be used in an approved clinical setting. Only psychiatrists (not psychologists, GPs, or other providers) can prescribe it. This is a narrower access pathway than Oregon's model.

The approved medicines: the TGA authorization covers MDMA (for PTSD) and psilocybin (for TRD). The psilocybin is produced under GMP (good manufacturing practice) standards — not prepared from mushrooms. Clinic pricing: approximately AUD $5,000-$15,000 for a full treatment course. Not covered by Medicare or private health insurance in most cases. This price point puts it out of reach for many Australians who most need it.

The treatment protocol: broadly follows the Hopkins/COMPASS clinical trial model. Two preparation sessions with the psychiatrist. One or two dosing sessions (typically 25mg) with monitoring. Integration sessions following. The psychiatrist manages the whole process — there's no separation between prescriber and facilitator as there is in the Oregon model.

The gap problem: the TGA authorization is a first step, not a complete solution. The narrow prescriber requirement (psychiatrists only), no Medicare coverage, and high out-of-pocket cost mean that access remains limited to people with financial resources and urban locations. Advocacy groups are pushing for expanded prescriber access and insurance coverage.
