I have episodic cluster headaches. I've read anecdotally that small doses of psilocybin can prevent cluster cycles. Is there any actual evidence for this? And what doses are people using?
Cluster headaches and psychedelics have an unusual evidence history. The anecdotal case series goes back decades — reports of sub-psychedelic doses of psilocybin or LSD terminating cluster cycles or extending remission periods. The mechanism hypothesis: psilocybin may affect the hypothalamic activity that drives cluster headache cycles (the hypothalamus is implicated in cluster pathology and responds to serotonin 2A receptor modulation). Yale published the first formal survey data in 2006 (Sewell et al.) examining 53 cluster patients who had used psychedelics — 22 of 26 psilocybin users reported period termination or remission extension. No controlled trial has been conducted, but this remains one of the most promising applications.
What the community reports: doses that work for cluster prevention are often sub-psychedelic (0.1–0.3g dried mushrooms taken every 5–7 days during or approaching a cluster period). The pattern that many describe is: take a sub-threshold dose at the first signs of a cluster cycle starting; repeat 1–2 more times over two weeks. This reportedly terminates the cycle or prevents it from starting. Higher doses (1–1.5g) are also reported to work for some people. The non-psychedelic dose aspect is medically interesting — it suggests the mechanism is not the altered state but direct receptor action.
Important caveat: 'Cluster Busters' (clusterbusters.org) is the primary community resource for this specific use. They have the largest informal evidence base and the most detailed protocols.
