Physical sensations during psilocybin: what's normal and what's concerning

I see a lot of posts from people panicking about physical sensations during experiences. Yawning, trembling, temperature changes, nausea, heart rate changes — what's expected and what should make you seek help?
Common and expected: yawning (serotonin-related), mild trembling or shivering (vasomotor effects, usually brief), temperature fluctuations (feeling alternately hot and cold), dilated pupils (standard), nausea in the first 30-60 minutes (usually passes), headache at the end of the experience. These are all within the normal range.
The nausea window: typically 30-60 minutes after ingestion. Fasting 4-6 hours beforehand helps significantly. Ginger tea before dosing helps many people. Lemon tek (acidic pre-conversion of psilocybin to psilocin) can actually INCREASE nausea onset speed but may reduce the duration.
When to seek help: sustained chest pain (not common with psilocybin but possible), severe vomiting with inability to keep water down, loss of consciousness, temperature above 103F. None of these are typical psilocybin effects — their presence suggests something else is happening or the substance is not what was thought.
